OJT MEMO
Hi OJTs,
I need a EMAIL (edang20@gmail.com) and SUBMIT in your Blackboard Account the following:
- Endorsement Form
- ALL WAIVER FULLY SIGNED by everyone (including ICEP)
- Certificate of Acceptance (COA)
- Training Plan (minimum six row)
- Trainee Schedule Sheet
- MOA
- Certificate of COMPLETION (COC)
- Company Evaluation
- Student (YOUR) Evaluation
- Final /Technical Report (in a correct FORMAT)
- TIME CARD and ID (if you have it photocopied)
- Photocopied letter of recommendation and student time schedule
- Learning Journals (in a correct FORMAT)
- ANY contracts or forms given by the OJT host company
- OTHER requirements/documents as given by the host company and University. (i.e. TOEIC, S2S Academy Certificate, other Seminar Certificates) ….. (any additional requirements) etc.
- ANY other OJT work related file made by the OJT or given by the company. (docs, excel, ppt, mp4, etc….)
If you have submitted any of these in advanced (maybe items #1 and # 5), kindly check (double check) with School’s Blackboard, email and collect to complete the requirements.
ITEM# 1 and 5 required before your START your OJT. (PRE-requirements)
NO SIGNED WAIVER DO NOT START OJT. NO signed MOA, NO OJT.
Deadline for submission (referring to the final requirements) is ALWAYS ONE week before the schedule FINAL EXAM. Week 10 of the current Term.
FAILURE to submit any one of the requirements, you will get a FAILING GRADE of 5 !
