OJT Send Supv Details

To OJT,

To validate your performance scores, ICEP, requires all OJTs to submit the details of your immediate supervisor, which as follows:

1. Name of the student
2. Student Number
3. Program (degree)
4. Student’s immediate supervisor’s name and job position (at the company)
5. Company email address of the Supervisor

If you have two or more supervisors, then submit all their details.
